Are you a compassionate and committed Social Worker or Nurse looking to make a meaningful impact in people's lives?
- You will be part of a dynamic team, working closely with clients, families, GPs, community nurses, specialists, Aboriginal Medical Services, and other care providers.
- Your advocacy, empathy, and ability to navigate systems will help clients access the support they need to live healthier, more empowered lives.
- Coordinate Care: Be the glue that holds it all together—supporting people with chronic and complex health needs through thoughtful, high-quality care coordination.
- Team Up for Tailored Support: Work together with clients, families, carers, and primary care teams to co-create care plans that actually fit—no cookie-cutter solutions here.
- Collaborate with the Best: Partner with GPs, nurses, and specialists to boost care planning and drive better health outcomes.
- Streamline the System: Link up with local health teams to smooth out the bumps and make sure services flow seamlessly for every client.
- Communicate with Meaning: Break down the jargon. Make info clear, useful, and accessible—and always listen for feedback to keep improving.
- A Degree in Social Work (with AASW membership) or othe relevant Allied Health Profession, You're a Registered/Enrolled Nurse (NMBA registered), or a qualified Aboriginal Health Worker—are welcome diverse pathways to care.
- Assessment Superpowers: You know how to dig deep—conducting holistic assessments, crafting care plans that actually work, and tracking outcomes that matter.
- Chronic Care Know-How: You get the complexity of chronic conditions and know how to navigate the system to get the best results for your clients.
- People Skills That Shine: You're a natural connector—able to build trust and communicate clearly with Aboriginal and Torres Strait Islander peoples, as well as those from culturally and linguistically diverse (CALD) backgrounds.
