In this role, you’ll work in close partnership with clients, families, GPs, community nurses, specialists, Aboriginal Medical Services, and other care providers to deliver coordinated, culturally responsive, and person-centred care. **What You’ll Bring to the Table**

You’ve got a degree in Social Work (with AASW membership) or othe relevant Allied Health Profession, you’re a Registered/Enrolled Nurse (NMBA registered), or a qualified Aboriginal Health Worker—we welcome diverse pathways to care.
