Home Care Clinical or Non-Clinical Care Partner

We are seeking a dedicated and compassionate professional to join our team as a Home Care Clinical or Non-Clinical Care Partner.

  • The successful candidate will work with clients in their homes to provide clinical or non-clinical care, including assessments, case management, and administrative duties.

This role requires strong communication and organisational skills, as well as the ability to work autonomously and as part of a team.

The ideal candidate will have previous experience in the home care package, community care or health/aged care sector, and hold a Bachelor's degree in Nursing or equivalent.

  1. Conduct comprehensive assessments of participants in their homes to identify their clinical and holistic needs.
  2. Develop and implement effective case management plans to support participants' wellbeing and promote independence.
  3. Monitor and communicate with participants on a regular basis to ensure their safety and wellbeing.
  4. Document all aspects of participant care, including goals, progress, and outcomes, using person-centred approaches.
  5. Undertake administrative duties, such as data entry, processing invoices, and responding to participant enquiries.
  6. Ensure that all evidence requirements are met, including quotes, confirmation of delivery of products and services.
  7. Work collaboratively with medical practitioners, allied health professionals, and other stakeholders to deliver high-quality care.
  8. Provide training, coaching, and guidance to Community Support Professionals, Enrolled Nurses, and Support Staff/Care Partner colleagues.
