Home Care Clinical or Non-Clinical Care Professional

We are seeking a dedicated and compassionate individual to join our team as a Home Care Clinical or Non-Clinical Care Professional.

About the Role:
  • This role involves providing high-quality care and support to individuals in their homes, ensuring their safety and well-being.
  • You will work closely with participants, their families, and other healthcare professionals to develop and implement personalized care plans.
Key Responsibilities:
  • Undertake comprehensive assessments of participants in their homes, identifying areas for improvement and developing strategies to address these.
  • Ongoing case management, supporting participants in meeting their clinical and holistic needs.
  • Regular monitoring and communication with participants and their families, reporting on progress and outcomes.
  • Documentation of participant goals, progress, and outcomes using person-centered approaches.
  • Administrative tasks, including data entry and processing invoices.
Requirements:
  • Advanced computer skills, including proficiency in software applications used in the role.
  • Strong administration and organizational skills, with ability to prioritize tasks and manage multiple projects.
  • Ability to work effectively in a fast-paced environment, managing competing demands and deadlines.
  • Excellent problem-solving and prioritization skills, with ability to think critically and make sound decisions.
