Care Coordinator Opportunity

We are seeking an experienced Care Coordinator to join our team at Right at Home Gold Coast. As a vital member of our team, you will oversee and facilitate the provision of high-quality care and services to our clients.

Key Responsibilities:
  • Conduct thorough client care assessments and develop comprehensive care plans
  • Manage client compliance and maintain a caseload of clients
  • Coordinate the delivery of personalized care tailored to clients' needs and care plans, ensuring adherence to allocated funding and budget requirements
  • Perform general administrative tasks associated with the role
Desired Skills:
  • Proven experience in aged care, NDIS, or mental health care coordination
  • Familiarity with My Aged Care, Home Care Package, and NDIS systems
  • Ability to evaluate and plan care needs within the community
  • Proficiency in using computer systems, including Microsoft Office 365, Sharepoint, Outlook, and rostering software
  • Excellent communication and interpersonal skills
  • Capacity for planning, organization, problem-solving, and solution-finding
  • Positive attitude with a strong aptitude for teamwork
  • Outstanding networking and consultation skills to establish relationships with external organizations for referrals
  • Exceptional customer service skills, both written and verbal, while maintaining confidentiality and privacy in accordance with company policies
Qualifications:
  • National Police Check (Compulsory)
  • Valid Australian Driver's License (Necessary)
