Job Description

We are seeking an experienced and motivated Clinical Care Coordinator to join our team at Coppin Centre Residential Aged Care Facility in Melbourne.

The successful candidate will be responsible for undertaking or supervising clinical care for individual residents, including complex wound care, pain management, behaviour management, end-of-life care and other clinical procedures as may be necessary.

Liaising with residential care staff, medical, allied health, hospital and other specialist health practitioners to ensure residents receive high level clinical care.

This role is ideal for a registered nurse who is passionate about delivering exceptional patient outcomes and working collaboratively as part of a multidisciplinary team.

Key Responsibilities:

  • Undertake or supervise clinical care for individual residents
  • Liaise with residential care staff, medical, allied health, hospital and other specialist health practitioners
  • Ensure residents receive high level clinical care
Required Skills and Qualifications

To be considered for this role, you must have:

Essential Requirements:

  • Current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia (NMBA)
  • Recent experience in caring for high care and dementia residents
  • Knowledge of wound care, palliative care, behaviour management, clinical assessments, care plans
