About the Role

Lives Lived Well is seeking a highly skilled and compassionate AOD Counsellor/Case Manager to join our team. As a key member of our multidisciplinary team, you will provide assessment, counselling, case management, and support services to a diverse range of clients.

Key Responsibilities
  • Conduct thorough assessments of clients' dependency and develop personalized treatment plans
  • Deliver individual and group counselling sessions, utilizing evidence-based therapeutic practices such as CBT, Mindfulness, DBT, and ACT
  • Provide case management and support services to clients, including coordination of care and linkage to community resources
  • Collaborate with other healthcare professionals to ensure comprehensive and coordinated care
  • Maintain accurate and confidential records of client interactions and progress
Requirements
  • Tertiary qualifications in social work, psychology, human services, occupational therapy, or other health-related disciplines
  • Ability to develop, facilitate, and evaluate therapeutic practices
  • Excellent relationship and rapport-building skills, with the ability to work with clients from diverse backgrounds
  • Strong verbal and written communication skills, with the ability to prioritize tasks and manage a caseload
  • A proactive team player, with a commitment to collaboration and continuous learning
