Case Manager Opportunity

This is a rewarding role where you will deliver strengths-based information, assessment, referral, advocacy, support, group work and brokerage assistance to young people aged 16 – 25 years.

You will work collaboratively with various stakeholders to ensure comprehensive support and effective outcomes empowering them to achieve independence and reach their full potential.

  • Key responsibilities include:
  1. Providing case management services to young people using a strengths-based framework;
  2. Working closely with stakeholders to identify and address the needs of clients;
  3. Developing and implementing strategies to support clients in achieving their goals;
  4. Collaborating with other organisations to provide a coordinated response to client needs;
  5. Maintaining accurate records and reporting on client progress;

The ideal candidate will have:

  • Relevant qualifications in human services or social sciences combined with direct experience working with high-risk/young people & families in a community-based environment;
  • Demonstrated understanding of trauma-informed, strengths-based practice and case management approaches, combined with strong youth engagement skills;
  • Able to manage crisis situations and make good decisions under pressure;
  • Ability to work respectfully and collaboratively with Aboriginal and Torres Strait Islander peoples, their families, communities, staff, and Aboriginal controlled organisations;
