Hours:Full-time, Monday to Friday, 8:30 AM - 4:30 PMReporting to:Care ManagerRemuneration:Competitive base salary + SuperannuationTravel Requirement: Covering Northern Rivers RegionKey Responsibilities:Conduct client care assessments and oversee ongoing care planningMaintain client compliance and manage a caseload of clientsCoordinate and deliver personalised care aligned with client needs, care plans, and allocated budgetsPerform general administrative tasks related to the roleSkills and Experience:Relevant qualifications (Cert IV or Diploma in Nursing highly regarded)Experience in aged care, NDIS, or mental health case management (preferred)Knowledge of My Aged Care, Home Care Packages, and NDIS frameworksProven experience in assessing and planning community-based careProficiency in Microsoft Office 365, SharePoint, Outlook, and rostering softwareExcellent verbal and written communication skills with strong customer service abilitiesAbility to handle incoming calls professionally and courteouslyStrong organisational, problem-solving, and decision-making skillsCollaborative team player with a proactive attitudeAbility to build strong relationships with external organisations for client referralsCommitment to confidentiality and privacy in line with company policiesWhy Join Us?
