About the Opportunity

As a Youth Homelessness Case Manager, you will be responsible for providing case management, outreach, and housing support to young people aged 15-25 who are at risk of or experiencing homelessness.

You will work collaboratively with various stakeholders, including homelessness access points and local networks, to deliver client-centered support that enhances the wellbeing and stability of young people.

Key Responsibilities:

  • Provide case management services to young people aged 15-25.
  • Advocate and liaise with housing access points and specialist services across the region.
  • Deliver services through a strengths-based, client-centered approach.
  • Build and maintain collaborative relationships with local networks and agencies.
  • Maintain accurate records and reporting in line with program requirements.
About You

To be successful in this role, you will possess:

  • A tertiary qualification in Youth Work, Social Work, or a related field.
  • Experience supporting young people experiencing or at risk of homelessness.
  • A strong understanding of the homelessness service system and youth-specific challenges.
  • Excellent communication, problem-solving, and advocacy skills.
  • A collaborative and flexible approach to working with diverse stakeholders.
