Job Description:

We are looking for a Registered Nurse to join our team as a Health Coach. In this role, you will provide remote patient care through coaching and education using digital communication tools. Your responsibilities will include providing telephone triage to patients with acute symptoms, working with patients and their caretakers to increase engagement and help them overcome barriers in their healthcare journey, guiding patients with their changing medical therapies using a clinician-designed protocol, connecting patients back with their clinicians as needed when their condition changes, and serving as the interface between the patients and others in the healthcare system.

Required Skills and Qualifications:

  • Current active Registered Nurse license in a compact state
  • 2+ years clinical experience in a specialty practice or acute care setting
  • Bachelor's or equivalent degree required
  • Highly proficient written/verbal communication and interpersonal skills to interact with patients, caregivers, and healthcare providers
  • Analytical skills required to problem solve patient issues and prepare reports
  • Strong customer service skills, with ability to identify problems and formulate solutions
  • Able to take on multiple tasks in a dynamic, fast-paced, and changing team environment
  • Passion for implementing technology to improve healthcare and able to quickly learn and use new software and tools
  • High-speed home internet access
  • Quiet home office space, free of distractions
