Social Services Professional

Western Health is seeking a highly motivated and dynamic Grade 3 Social Worker to join our multidisciplinary team at Dame Phyllis Frost Centre (DPFC), a maximum security women\u2019s prison.
This role forms part of a well-supported and innovative team, and the successful applicant will be onboarded with orientation and training to the custodial setting, including additional training to support the delivery of culturally appropriate, trauma-informed, gender-affirming care for all patients. This role is both operationally and clinically supported by Western Health.
The Grade 3 Social Worker role within DPFC involves the assessment and intervention to a range of populations, across such issues as family violence, child protection, women\u2019s health, grief and loss, aged and disability.
This role offers the opportunity for development in leadership and creative problem-solving, with a mix of acute and subacute work.
Key Responsibilities
- Assess and intervene with a range of populations, including those experiencing family violence, child protection, women\u2019s health, grief and loss, aged and disability.
- Develop and implement culturally appropriate, trauma-informed, gender-affirming care plans for patients.
- Collaborate with multidisciplinary teams to provide comprehensive care and support to patients.
- Participate in ongoing training and professional development to enhance skills and knowledge.
- Contribute to the development of innovative solutions to complex care challenges.
