">The Acute to Aged Related Services team provides specialised care for older patients in acute inpatient settings who have complex care needs. This service is designed to deliver patient-centred, comprehensive assessment, care planning, and coordination to enable safe and timely discharges.
">As a Clinical Nurse Specialist 2, you will provide advanced clinical care, leadership, and expertise in managing complex cases. A key part of your role will be delivering the first face-to-face assessment within one working day of referral, then working with families, carers, and service providers to develop safe discharge plans.
">You will balance competing priorities, manage patients exhibiting challenging behaviours, and advocate for older patients while guiding and supporting your colleagues. Each day will bring opportunities to demonstrate clinical leadership, apply advanced assessment and case management skills, and contribute to improving outcomes for older people.
">Key Responsibilities:
">- Provide advanced clinical care, leadership, and expertise in managing complex cases">
- Deliver the first face-to-face assessment within one working day of referral">
- Work with families, carers, and service providers to develop safe discharge plans">

Requirements:
">To be successful in this role, you will need:
">- Current registration as a Registered Nurse with the relevant nursing authority">
- Relevant post-registration qualifications and experience appropriate to the role of CNS2">
