Job Title: Advanced Care Specialist

We are seeking an experienced and skilled Clinical Nurse Specialist 2 to join our Acute to Aged Related Services team.

About the Role:

As a key member of our interdisciplinary team, you will provide expert clinical care and leadership in managing complex cases for older adults. Your advanced assessment and case management skills will play a vital role in improving patient outcomes.

Main Responsibilities:

  • Develop and implement comprehensive discharge plans with families, carers, and service providers to ensure timely and safe transitions.
  • Utilize your expertise to balance competing priorities, manage patients exhibiting challenging behaviours, and advocate for the needs of older adults while guiding colleagues.
  • Contribute to collaborative efforts that promote patient-centred care and efficient discharges.

Requirements:

  • Holding current registration as a Registered Nurse with AHPRA.
  • Possessing relevant post-registration qualifications and experience aligned with the CNS2 role.
