Job Summary

We are seeking a highly skilled and compassionate Registered Nurse to join our team at Right at Home Perth Northern Suburbs & Perth West Coast. As a Nurse Care Coordinator, you will play a vital role in delivering high-quality, person-centered care to our clients in the comfort of their own homes.

Key Responsibilities
  • Clinical Leadership: Provide clinical guidance and supervision to non-clinical team members in a community setting, ensuring the highest standards of care are maintained.
  • Care Planning and Coordination: Develop and implement individualized care plans, taking into account the unique needs and preferences of each client.
  • Client Assessments: Conduct thorough assessments of clients' physical and emotional needs, with a focus on Consumer Directed Care principles.
  • Service Delivery: Coordinate the provision of quality, person-centered care, ensuring that services are delivered in accordance with client needs, service plans, and available funding.
  • Clinical Services: Provide a range of clinical services, including care plan assessments, medication management, incontinence care, wound care, chronic disease management, complex wound management, palliative care, and health and diabetes management.
  • Administrative Tasks: Manage documentation related to client care, ensuring compliance with AHPRA regulations and legislation.
  • On-Call Duties: Participate in an on-call roster, providing support and guidance to clients and team members as needed.
Requirements
  • Qualifications: Current registration as a Registered Nurse with AHPRA, First Aid & CPR certification, Australian National Police check, Professional Indemnity Insurance, and a valid Driver's Licence.
  • Experience: Proven experience in assessing and planning care needs within the community, with a strong understanding of Consumer Directed Care principles.
  • Skills: Excellent communication and interpersonal skills, with the ability to work autonomously or as part of a team. Strong time management and organizational skills, with a commitment to ongoing professional development.
