About the Role:

We are seeking a qualified Registered Nurse to join our team at Right At Home Perth Northern Suburbs & Perth West Coast. As a Nurse Care Coordinator, you will be responsible for delivering high-quality, person-centered care to our clients in their own homes.

Key Responsibilities:

  • Provide clinical delivery and supervision of non-clinical team members in a community setting
  • Process intakes, conduct care assessments, and develop care plans in accordance with Consumer Directed Care principles
  • Coordinate the provision of quality care services, including medication management, wound care, and palliative care
  • Conduct general administration tasks, such as documentation and reporting, in alignment with AHPRA regulations and legislation
  • Occasionally participate in on-call duties

Requirements:

  • Current AHPRA registration as a Registered Nurse
  • First Aid & CPR certification
  • Australian National Police check
  • Professional Indemnity Insurance
  • Valid Driver's Licence and reliable, presentable vehicle
  • Smart mobile phone (iPhone or Android)

Preferred Qualifications:

  • Cert IV in Training and Assessment
  • Exceptional communication and interpersonal skills
  • Able to work autonomously or within a team environment
  • Excellent time management and organization skills
  • Commitment to ongoing professional development
