Primary Health Care Coordination Opportunity

We are partnering with the Adelaide PHN and Hutt Street Centre to enhance primary care services for individuals experiencing homelessness.

This is an exciting opportunity for a dedicated professional to work alongside our General Practitioners who provide weekly visiting clinics at Hutt Street Centre in the Adelaide CBD.

Key Responsibilities
  • Provide general practice coordination with a focus on chronic disease management and patient care
  • Conduct comprehensive health assessments and develop effective care plans
  • Liaise with healthcare professionals and support networks to ensure seamless patient care
  • Assist with administrative tasks and contribute to a collaborative team environment
Essential Qualifications
  • AHPRA registration is mandatory
  • Experience in primary health care coordination or a related field is required
