Job Title:

A highly skilled coordinator is sought to provide exceptional care and support services to clients in their own homes.

Key Responsibilities:
  • Ensure seamless transitions to appropriate services by auditing eligibility and coordinating client referrals.
  • Deliver a client-centered onboarding experience and ongoing care support service.
  • Build rapport with clients, families, referrers, and staff to ensure excellent service delivery.
  • Provide outstanding customer service, handling sensitive interactions with respect and care.
About the Role:

The ideal candidate will possess passion, purpose, and a drive for excellence, with skills including coordination and client referral experience, excellent customer service, strong administration, and interpersonal communication.

  • Coordination and client referral experience in Aged Care or Hospital/Community settings is desirable.
  • Ability to thrive in dynamic, fast-paced environments, working autonomously or collaboratively.
  • High-level computer skills.
