About the Role:
The position of Care Coordinator is responsible for delivering safe, high-quality clinical nursing care to residents within the scope of practice as determined by relevant regulatory bodies.
On a day-to-day basis this role shall report to the Care Manager and General Manager.
- Create comprehensive histories from residents and/or family members to identify care needs by participating in resident admissions and ongoing assessments.
- Maintain accurate documentation which reflects care delivery.
- Develop, implement and regularly evaluate resident care plans as directed and assist with resident admission assessments.
- Provide direct clinical care to residents as required, including personal hygiene, massage therapy, pain management and GP rounds.
- Administer medications to residents and assume responsibility for correct ordering, storage and documentation procedures in accordance with organisational policies and procedures.
Key Requirements:
- Current registration with relevant professional regulatory bodies.
- Knowledge of Residential Classification Scale and Standards for Aged Care.
- Current National Police Certificate (or ability to acquire).
- NDIS Workers Check
- Knowledge of government standards required for aged care delivery.
- An understanding of the Commonwealth Aged Care Funding Instrument.
- Strong clinical and documentation skills.
- An understanding of aged care accreditation process.
