About the Opportunity
The Consultant for Community Empowerment plays a vital role in empowering clients and their carers to thrive within their communities. Focusing on those experiencing social isolation, mental health conditions or severe self-neglect, you will use your skills to help them achieve wellness goals that enhance their quality of life.
Through comprehensive case management, you will guide clients through complex community and healthcare service networks, working collaboratively to establish and achieve their wellness objectives.
As a Consultant for Community Empowerment, you will ensure clients and their carers receive coordinated support that fosters independence and builds meaningful community connections.
About You
As the Consultant for Community Empowerment, you will combine clinical expertise with cultural competency to advocate effectively for clients while delivering culturally safe care that respects diverse backgrounds.
Your excellent communication and relationship-building abilities will enable you to build strong rapport with clients from varied communities, while your empathy and resilience guide all interactions.
You will demonstrate analytical skills to assess complex social situations, develop appropriate interventions, and monitor client progress, working effectively under pressure and collaborating seamlessly with multidisciplinary teams.
Essential Qualifications
- Relevant qualifications in Human Sciences, Allied Health or Community Services fields
- Proven experience in case management and supporting informal carers
- Minimum 5 years' work experience within human services settings
Desirable Skills
- Experience working with clients living with mental health conditions
- Understanding of contemporary models of care for managing severe self-neglect
