We are seeking a dedicated and passionate Youth Residential Case Manager to join our team. As a key member of our team, you will be responsible for providing intensive case management support to young people aged 16-21 who are experiencing homelessness and have complex needs.

Your Key Responsibilities:

  • ">
  • Support young people with complex issues to make positive life changes">
  • Respond to referrals from internal and external support services">
  • Provide evidence-informed case management sessions that are person-centred">
  • Create holistic and strength-based individualised support plans">
  • Attend network and external agency case meetings to strengthen service delivery">
  • Assist clients in transitioning out of the service into independence or other services">

Required Skills and Qualifications:

To be successful in this role, you will need:

">
  • ">
  • A Diploma in Social Welfare (or higher) or relevant industry experience">
  • Experience working with vulnerable young people with mental health challenges">
  • Case Management experience working with young people with complex needs">
  • An understanding of Mental Health challenges experienced by young people">
  • Demonstrated experience of trauma informed practice">
  • Strong communication skills to assist in day-to-day client support">
  • High level of record keeping and administrative skills">
