We're seeking a dedicated Enrolled Nurse to join our team as an Age Care Specialist. This role involves conducting personalized assessments for new and existing Home Care Package clients, developing clear care plans in collaboration with clients and their representatives, and recommending tailored services and supports to empower clients to maintain independence.

  • Conduct thorough assessments of client needs using an enablement and wellness approach.
  • Collaborate with clients and their support networks to develop effective care plans that align with their lifestyle, preferences, and goals.
  • Recommend targeted services and supports to enhance client well-being and quality of life.

As an Age Care Specialist, you'll work independently and as part of a multidisciplinary team to support each client's unique needs. You'll have the opportunity to grow professionally and develop your skills in this rewarding role.

Key Responsibilities:
  • Conduct assessments of client needs through targeted questioning and medical history reviews.
  • Develop and implement comprehensive care plans that address client goals and objectives.
  • Communicate effectively with clients, families, and healthcare professionals to ensure seamless care coordination.
  • Maintain accurate and up-to-date documentation within our care management system.
