About the RoleThe Clinical Care Partner role involves providing clinical oversight and management to enable older adults to live safely and independently at home.This includes coordinating tailored, responsive, and inclusive support with participants, their supporters, and interdisciplinary teams within a rights-based framework.

Key responsibilities include developing individualised care plans, incorporating participant goals, service preferences, cultural needs, risk factors, and environmental requirements into care planning and delivery.

The Clinical Care Partner establishes and updates care plans, ensuring participants understand available services and works collaboratively with multidisciplinary teams to provide clinical care management and guidance.

To be successful in this role, you will require a Bachelor's degree in nursing, social work, allied health, or similar field, along with demonstrated experience in care coordination, case management, or similar roles in aged care, health, or disability services.

You should also possess a solid understanding of the Support at Home Program, Quality Standards, and aged care rights, as well as competence in interpreting clinical guidance and escalating appropriately.

  • Clinical Care Management: Develop and implement individualised care plans that cater to the unique needs and goals of participants.
  • Collaboration and Teamwork: Work closely with interdisciplinary teams to provide comprehensive support and guidance to participants.
  • Care Coordination: Coordinate resources and services to ensure participants receive timely and effective support.
