Mental Health Service Coordinator

We are seeking a highly skilled and dedicated Mental Health Service Coordinator to join our team at the Northern Adelaide Local Health Network (NALHN). As a key member of our team, you will play a vital role in the development, implementation, and evaluation of quality non-government services for consumers of the NALHN.
Key Responsibilities- Facilitate the identification and referral of consumers in acute/sub-acute settings into appropriate non-government psychosocial programs.
- Identify and refer consumers into other programs where appropriate.
- Work with staff to ensure consumers are identified, allocated, provided with care coordination, referred to the appropriate non-government organisation, reviewed, exited or transferred as appropriate to longer term support services where required.
- Appropriate degree or equivalent qualification for one of the following Allied Health disciplines: Occupational Therapist, Clinical Psychologist, or Social Worker.
- Registration with the relevant professional body.
- Experience working in mental health services.
