Deliver Exceptional Care

We are seeking a highly skilled and dedicated Care Manager to join our team. As a Care Manager, you will play a critical role in delivering exceptional care to elderly Australians, ensuring their safety and well-being at home for longer.

Key Responsibilities:

  • Deliver high-quality client care, tailored to individual needs and preferences.
  • Oversee and manage the delivery of quality care services, ensuring compliance with relevant standards and regulations.
  • Develop, monitor, and reassess care plans in collaboration with clients, their families, and healthcare professionals.
  • Work closely with internal teams to facilitate smooth client on-boarding and lead conversion processes.

You will have the opportunity to make a real difference in the lives of our clients, while contributing to our organization's mission of providing exceptional care services.

Requirements:

  • Experience in aged care, with knowledge of Home Care Packages, Commonwealth Home Support Program, and Aged Care Quality Standards.
  • Frontline management skills, with ability to travel and conduct face-to-face client meetings.
  • Health-related qualifications not essential but highly regarded.
  • Procura program experience desirable.
