Job Summary: We are seeking a dedicated and compassionate Disability Support Worker to join our team. As a Disability Support Worker, you will have the opportunity to make a positive impact in the lives of people with disabilities, providing them with the care and support they need to thrive.

Key Responsibilities:

  • Follow individual support plans, behaviour support plans, and health care plans;">
  • Participate in community activities within a supported independent living environment;">
  • Maintain and monitor a client's support plan whilst modelling a person-centred approach;">
  • Identify and support a range of learning opportunities for our clients;">
  • Observe and document an individual's progress and any changes through detailed note-keeping.">

Requirements: To be considered for this role, you will need to possess the following qualifications and experience:

  • ">
  • Certificate III or above in Individual Support (Disability or Aged Care);">
  • National Police Check Clearance;">
  • Working with Children Check (Known as Blue Card);">
  • NDIS Worker Clearance Certificate or Yellow Card;">
  • Experience in working with people with challenging behaviours;">
  • Experience in mental health;">
  • Experience in safe handling of food;">
  • Experience in safe use of Medication;">
  • Certificate of NDIS Orientation Module;">
  • COVID-19 Vaccine Certificate (Fully vaccinated);">
  • 2 x References;">
  • Current QLD Driver's License;">
  • Comprehensive Car Insurance.">
