Job Description:We are seeking a dedicated and compassionate Registered Nurse to join our team. As a key member of our care management coordination role, you will play a vital part in delivering high-quality clinical care services to complex Home Care Package clients.
You will be responsible for building and maintaining strong relationships with clients, carers, and families, as well as developing, implementing, and reviewing individualised care plans to meet client needs.
Key aspects of this role include conducting assessments to identify client requirements, ensuring compliance with relevant standards, and providing in-home clinical services such as medication management, wound care, chronic disease management, and palliative care.
You will also act as a liaison between referral sources, families, and the care team to ensure seamless communication and care delivery.
The ideal candidate will possess excellent written and verbal communication skills, with a talent for building relationships and achieving positive outcomes.
They will also have a proactive approach to problem-solving, strong organisational skills, and a minimum of 2 years' experience in a care management/coordination role within aged care or community care.
Strong interpersonal and communication skills are essential, along with above average time management and organisational skills.
A knowledge of the aged care and disability sectors is highly desirable.
