About the Role

We are seeking a motivated and empathetic NDIS Relationship Manager to join our organization.

This is a permanent, full-time opportunity based out of Melbourne with work-from-home flexibility and travel to visit clients as required.

  • Manage a meaningful caseload of NDIS participants – tailor services to meet their needs, goals, and preferences
  • Conduct home visits and participant reviews – because real care happens where it matters most
  • Keep participant documentation sharp, accurate, and always up to date
  • Collaborate closely with internal teams and external partners to deliver the best outcomes
  • Provide ongoing assessments and adapt care plans as needed
  • Support the wider team with operational needs

You are a driven professional keen to develop your skill set. You have a genuine desire to provide high-quality service to all stakeholders and are comfortable dealing with multiple priorities.

Your information technology skills are evolving, and you are comfortable adapting to new systems and processes. Ultimately, you enjoy being challenged and contributing to a positive working culture.
