Job Opportunity

Ozcare seeks a dedicated professional to lead their Home Care Package business. This role involves overseeing a team in planning, managing, and delivering client care and support across various programs.

Your Key Responsibilities:

  • Provide leadership to the team in managing and coordinating care services to ensure high standards.
  • Develop and implement tailored care plans that cater to individual client needs.
  • Liaise with clients, families, and healthcare professionals to deliver comprehensive care.
  • Foster strong relationships with clients, stakeholders, and team members while assisting clients in meeting their unique needs and goals.
  • Ensure compliance with relevant legislation and organizational policies.
  • Monitor and evaluate care services to continuously improve quality and efficiency.

Requirements for the Role:

  • Demonstrate experience in case management and client care coordination in a Community Aged Care setting.
  • Display strong verbal, written, and interpersonal communication skills to communicate effectively with diverse individuals.
  • Show ability to develop staff, foster teamwork, and work collaboratively as a team member while providing leadership.
  • Provide proof of immunity or willingness to become vaccinated against MMR, Varicella, and Pertussis.
  • Hold a current First Aid and CPR Certificate.
  • Current National Police Check prior to commencement.
  • Hold a current Queensland driver's licence.
  • Provide tertiary qualifications and/or relevant experience in a health-related field (e.g., Health and Human services).
