Leadership Role in Care Management

About the Position

As part of our multidisciplinary team, you will work closely with the General Manager to oversee day-to-day clinical operations and lead a team of Support Workers. Your role will focus on providing high-quality, professional care management advice and assistance to the General Manager of Operations and drive a positive and supportive team culture.

Key Responsibilities Include:
  • Providing expert care management advice and support to the General Manager and DCR Principals.
  • Overseeing care management for clients with varying disabilities and/or trauma within the Disability and Child Safety space.
  • Writing clear and concise care plans for Support Worker implementation.
  • Providing regular Support Worker supervision on home visits.
  • Ensuring timely, factual, and compliant incident reporting.
  • Liaising with other health professionals on client care plans to ensure individualized support.
  • Ensuring compliance and safety across SIL housing, support workers, records, and documents.
  • Completing Client Needs Assessments monthly and as required.
  • Participating in an after-hours' contact roster to provide professional advice on critical incidents and client care matters.
