Nurse - Community Healthcare Specialist

A high-performing clinical nurse is required to deliver exceptional patient-centred care in a community-based setting.
As part of the CALHN Urgent Care Hub team, you will play a vital role in providing rapid, multidisciplinary assessment and acute care for patients with complex health needs.
Working closely with a dynamic multidisciplinary team, you will provide expert nursing care including assessment, clinical interventions, and care coordination in a clinic setting.
Key responsibilities include:
- Providing high-quality patient care to individuals with complex health needs
- Collaborating effectively with a broad network of partners to ensure seamless integrated care
- Contributing to quality improvement initiatives and supporting the professional development of colleagues
The ideal candidate will possess strong clinical expertise, leadership ability, and excellent communication and problem-solving skills.
Requirements
Registered or eligible for registration with the Nursing Board of Australia, holding or eligible to hold a current practising certificate
Minimum 3 years' post-registration experience, with proven competence in nursing practice and clinical assessment relevant to the role
Strong communication, problem-solving, and negotiation skills, with the ability to adapt to changing environments and manage competing priorities
