Job Description:
We are seeking a highly skilled and compassionate Welfare Support Coordinator/Intake Officer to join our team. As a key member of our support/care team, you will be responsible for coordinating care plans, conducting comprehensive client assessments, and promoting quality of life for individuals in need.
Responsibilities:
- Liaise with all members of the support/care team to provide coordinated care and support services.
- Carry out thorough and accurate client assessments to identify their needs and develop tailored care plans.
- Contribute to the development, implementation, monitoring, and review of care plans to ensure they meet clients' needs and promote quality of life.
- Promote a consultative and collaborative approach to client care planning and quality-managed activities with external stakeholders, including welfare and community service agencies, private entities, and government bodies.
- Monitor and report on clients' progress, maintaining accurate records and documentation.
- Support and assist clients in developing life skills, mental well-being, and reaching individual goals within the community.
- Refer clients to relevant agencies for additional support when necessary.
- Assist with the orientation of new staff and ongoing professional development of staff members.
Essential Skills and Qualifications:
- Current First Aid certification is essential.
- A relevant advanced diploma or higher qualification in community service or community development is required.
- Membership of the Australian Community Workers Association (ACWA) is desirable.
- Demonstrated knowledge and skills in assessment, treatment, and evaluation are essential.
- Ability to work effectively in a multidisciplinary team setting as well as autonomously is crucial.
- Capacity to work with people from diverse backgrounds with compassion, empathy, and without judgment is vital.
- Demonstrated experience and knowledge of the National Disability Insurance Scheme (NDIS) and working within a legislative framework are essential.
Desirable Attributes:
- A can-do attitude, enthusiasm, and excellent communication skills are highly desirable.
- Excellent prioritisation and organisational skills are essential for this role.
- Proficiency in a second language other than English is an advantage.
