We are seeking an experienced Occupational Therapist to join our team as a Community Care Specialist. As part of this role, you will work autonomously with clients in their homes, providing assessments and interventions to improve their daily routines, independence, and overall quality of life.
Job Description:- Deliver home-based occupational therapy assessments and interventions for clients with Home Care Packages or NDIS funding.
- Develop and implement personalized therapy programs to address clients' needs and goals.
- Collaborate with multidisciplinary teams to provide comprehensive support for clients.
- Review and evaluate the effectiveness of therapy programs, making adjustments as needed.
- Relevant degree in Occupational Therapy.
- Current registration with AHPRA.
- Excellent communication and interpersonal skills.
- Ability to work independently and prioritize tasks effectively.
