Senior Social Worker Role

This is a challenging and rewarding role for an experienced Senior Social Worker to join our dynamic team.

Key Responsibilities:

  • Provide specialist social work services to keep consumers and their carers well in the community.
  • Reduce hospital admissions and facilitate safe, early discharges from hospital.
  • Ensure the provision of optimal health outcomes consistent with policies and procedures.
  • Participate in professional development, clinical supervision, teaching, quality activities and other relevant departmental and administrative duties.

Requirements:

  • A Social Work degree providing eligibility for membership of the Australian Association of Social Workers (AASW).
  • Extensive post graduate experience working as a Social Worker with community outpatient and/or people with chronic diseases.
  • Demonstrated high level organisational and time management skills, along with excellent written and verbal communication skills.
  • Australian C-Class driver's license required.
