Primary Care Physician

Deliver exceptional primary healthcare services to young people as part of a collaborative and holistic team that values diversity and inclusivity.

Responsibilities:
  • Conduct thorough assessments for individuals experiencing physical and mental health issues, providing high-quality care that meets their needs.
  • Collaborate with internal and external stakeholders to provide comprehensive care, ensuring seamless transitions and optimal outcomes.
  • Develop Mental Health Care Plans for internal referrals and Allied Health Services, fostering a supportive environment for patients to thrive.
  • Provide accessible and inclusive primary healthcare services to young people, addressing their unique health concerns and promoting overall well-being.
  • Respond to medical/health concerns presented by patients, including diagnosis, investigation, treatment, and referral as appropriate, prioritizing patient-centered care.
  • Encourage young people to engage with sexual health and drug and alcohol services, as well as preventative healthcare initiatives, promoting healthy lifestyles and disease prevention.
  • Have access to clinical supervision from the clinical manager, ensuring ongoing professional development and support.
Essential Criteria:
  • Approved tertiary qualifications in a relevant discipline, with current registration with AHPRA, demonstrating expertise and commitment to the field.
  • Strong clinical experience in youth health and mental health services, showcasing ability to navigate complex cases and deliver effective interventions.
  • Proven ability to apply evidence-based practices in therapeutic settings, promoting best practices and optimizing patient outcomes.
  • Experience in coordinating care for young people with diverse health needs, highlighting capacity to manage multiple priorities and ensure comprehensive support.
  • Capacity to work effectively within a multidisciplinary team, fostering collaboration and driving positive change.
