NDIS Support Coordinator Role

This critical position within our Services Team provides comprehensive support to clients, focusing on assessment, care planning, monitoring, and referrals. The Support Coordinator plays a vital role in enhancing participants' knowledge and experience, helping them establish meaningful connections within their community.

To achieve this, the ideal candidate will implement strategies to help participants build capacity and achieve their plan goals. This includes coordinating and assessing supports and service providers to meet specific needs and goals.

Main Responsibilities:

1. Client Engagement & Support:

• Develop and implement strategies to enhance participants' capacity and achieve plan goals.
• Coordinate and assess supports and service providers to meet specific needs and goals.
• Build the capacity of NDIS participants and their families to navigate local service systems effectively.

2. External Engagement & Networking:

• Participate in forums, networking events, and new business activities to elevate our organization's reputation and visibility.
• Adhere to NDIS Code of Conduct and Quality and Safeguarding guidelines, ensuring exceptional customer service experiences.
• Foster relationships with healthcare professionals and community organizations to facilitate coordinated care.

3. Documentation & Compliance:

• Maintain accurate documentation to comply with policies and procedures.
• Work within legislated NDIS frameworks, adhering to safety regulations and best practices.
• Contribute to quality processes and manage client feedback to drive continuous improvement.

4. Professional Development & Growth:

• Engage in regular professional development activities to enhance skills and knowledge.
• Develop and maintain relationships with community organizations for holistic case management.

5. Business Development & Growth:

• Establish and maintain strong relationships with referral sources, identifying new opportunities for growth.
• Retain existing consumers while achieving KPIs related to new and existing consumer engagement.
