Home Caring is a reputable and fast-growing provider of NDIS and Aged Care services, nationwide. We are currently seeking a passionate and experiencedCase Manager to join our team and support a growing number of clients in the Acacia Ridge and surrounding area.

This role is ideal for someone who is community-minded, culturally sensitive, and committed to providing high-quality, person-centred care. You'll be working with clients across both Aged Care and Disability sectors.

Key Responsibilities
  • Conducting client assessments and developing person-centred care/support plans
  • Coordinating services, monitoring outcomes, and reviewing care plans regularly
  • Advocating for clients and ensuring their cultural, linguistic, and health needs are met
  • Supporting clients to access appropriate services and resources
  • Liaising with families, support workers, and health professionals
  • Ensuring compliance with NDIS and Aged Care quality standards
  • Enrolling and management of Home Care Packages for Aged Care and NDIS clients
What We're Looking For
  • Experience in aged care, disability, or community health settings
  • Strong communication, problem-solving, and organisational skills
  • Understanding of NDIS and/or Aged Care systems
  • Empathy, cultural awareness, and a passion for helping others
  • Current First Aid & CPR, National Police Check, and NDIS Worker Screening
  • VIC Driver's Licence and reliable vehicle
  • Full working rights in Australia
  • Ability to assist the growth of the business by sourcing and signing up clients
