We are currently seeking a passionate and experiencedCase Manager to join our team and support a growing number of clients in the Acacia Ridge and surrounding area.
This role is ideal for someone who is community-minded, culturally sensitive, and committed to providing high-quality, person-centred care. You'll be working with clients across both Aged Care and Disability sectors.
Key Responsibilities- Conducting client assessments and developing person-centred care/support plans
- Coordinating services, monitoring outcomes, and reviewing care plans regularly
- Advocating for clients and ensuring their cultural, linguistic, and health needs are met
- Supporting clients to access appropriate services and resources
- Liaising with families, support workers, and health professionals
- Ensuring compliance with NDIS and Aged Care quality standards
- Enrolling and management of Home Care Packages for Aged Care and NDIS clients
- Experience in aged care, disability, or community health settings
- Strong communication, problem-solving, and organisational skills
- Understanding of NDIS and/or Aged Care systems
- Empathy, cultural awareness, and a passion for helping others
- Current First Aid & CPR, National Police Check, and NDIS Worker Screening
- VIC Driver's Licence and reliable vehicle
- Full working rights in Australia
- Ability to assist the growth of the business by sourcing and signing up clients
