Key Responsibilities:- Assist patients and their families with navigating the complexities of aged care placement and support their safe return to community living.
- Work closely with resource providers, negotiating and coordinating necessary services to ensure a smooth transition for each patient.
- Maintain up-to-date knowledge of available options and liaise with both facilities and families.
- Independent & Initiative-Driven: Able to work autonomously and advocate for patients with various organisations and departments.
- Effective Communication & Interpersonal Skills: Strong communication skills with ability to relate to clients from diverse backgrounds and support those in distress.
- Experience in Aged Care & Hospital Settings: Previous experience working with patients and families in aged care or hospital environments, particularly in placement coordination.
- Salary AHA-2 $58,709 - $63,154 p.a. + Superannuation and Salary Sacrifice Benefits
