Our key industrial, commercial and service centre for the shire and surrounding region is Colac, with a population of 12,000. Apollo Bay is the other major urban centre with a permanent population of 1,000 that swells to over 15,000 during the summer season.
Job Description: Home Care Support Professional- Support people of all ages and abilities with personal care, domestic tasks, transport, participating in social activities and maintaining community connections; with a focus upon capacity building, restorative care, social inclusion and maintaining or promoting a person's capacity to live as independently and autonomously as possible.
- Providing in-home and community-based support for personal care, respite support and domestic duties.
- Supporting clients with daily living activities, including personal care, meal preparation and housekeeping.
- Assisting clients with transportation to appointments and social events.
- Maintaining accurate records of client interactions and progress.
- Collaborating with multidisciplinary teams to ensure comprehensive care and support.
- A Victorian Drivers licence and access to a registered, comprehensively insured and roadworthy motor vehicle that can carry passengers.
- Previous experience working with people of all ages and abilities routine health needs.
- Excellent communication skills and an ability to work with a diverse range of people and age groups.
- Clear understanding and ability to implement duty of care and confidentiality principles.
- Well-developed organisational time management and problem-solving skills with a demonstrated ability to work with a minimum of supervision.
The estimated annual salary for this role is $59,904.02 to $61,334.05 per annum pro rata plus superannuation. In addition to the hourly rate, you will be reimbursed for client related travel kilometres. We also offer a job where you can make a real difference and meaningful contribution to people's lives.
