Job Overview

Mental health support workers play a vital role in helping individuals with low to moderate mental health needs avoid re-admission or minimize the potential for hospital admission.

Key Responsibilities
  • Work independently and as part of a multidisciplinary team to provide support to consumers, engaging them through structured 12-week programs using goal-based recovery models.
  • Offer generalized and short-term supports to help consumers build resilience and stay engaged with their recovery journeys.
  • Conduct initial assessments and engage with consumers weekly, facilitating recovery planning and providing ongoing community-based support with a transition-based approach.
  • Collaborate closely with external clinical teams, maintaining accurate documentation and records using CareLink and EMR to ensure risk assessments, care plans, and progress notes are up-to-date.
About You
  • Possess a minimum Certificate IV in Mental Health or Community Services.
  • Demonstrate a commitment to mental health recovery and the ability to use strengths-based approaches to empower consumers.
  • Exhibit strong interpersonal skills to engage with clients and collaborate with external clinical teams, as well as the ability to advocate effectively.
  • Display a good understanding of local resources and community access.
