Role Overview

We are seeking an experienced Intake Support Coordinator to join our team. As a key member of our support services team, you will play a vital role in ensuring the seamless onboarding of clients from the My Aged Care portal.

About the Role:

  • You will be responsible for initial contact with clients, screening and assessment, and ensuring all functions occur in accordance with our mission, objectives, and priority outcomes.
  • Responsibilities include processing aged care client intakes and referrals in a timely and accurate manner, liaising with relevant Managers and Coordinators, and completing required documentation as requested.
  • You will also build relations with key stakeholders, Regional Assessment Services (RAS), My Aged Care (MAC) and other service providers to enhance our suite of services.

About You:

  • Demonstrated experience and knowledge of working in a client intake and customer/client services environment.
  • Must be tech-savvy and able to navigate databases confidently.
  • Able to network effectively and collaborate with professionals from various backgrounds, including referral agencies.
  • Work independently, be self-motivated, and effective as part of the support services team.

Qualifications:

  • Relevant qualification in a healthcare field or transferable skills/experience.
  • Current National Police Certificate clearance and willingness to renew it every 3 years as part of ongoing employment.
  • Open Driver's Licence and own car.
