Community Rehabilitation Specialist

The successful candidate will be responsible for facilitating the transition of clients from hospital to home and promoting community reintegration.
This is a key role within our multidisciplinary team, which actively engages with people to improve and maintain health, independence, and well-being.
We coordinate and provide integrated care across multiple settings within the local community.
About the Role- Complete comprehensive assessments and care planning in collaboration with clients and interdisciplinary teams
- Facilitate the successful transition of clients from hospital to home and toward community reintegration
- Coordinate care plans to ensure high-quality client-centred care
- Support client flow within the community rehabilitation program
- Entry-level Allied Health qualification or equivalent in line with relevant discipline requirements
- Current registration with the Australian Health Practitioner Regulation Agency (AHPRA)
- Current Driver's Licence
- Working with Children's Check
- Relevant clinical and leadership experience
