About the Role

We are seeking a highly skilled Registered Nurse to join our clinical team at Southern Cross Care's Residential Aged Care Homes. As a Registered Nurse, you will play a vital role in providing exceptional care and support to our residents, working collaboratively with our multidisciplinary team to deliver high-quality, person-centered care.

Key Responsibilities

  • Provide clinical leadership and guidance to the care team, ensuring the delivery of high-quality, person-centered care.
  • Develop and implement individualized care plans, taking into account the unique needs and preferences of each resident.
  • Collaborate with medical practitioners, family members, and other healthcare professionals to ensure seamless care coordination.
  • Conduct regular assessments and evaluations to identify areas for improvement and implement changes as needed.
  • Supervise and mentor junior staff members, providing guidance and support to ensure they have the skills and knowledge needed to provide high-quality care.
  • Participate in quality improvement initiatives, contributing to the development and implementation of new policies and procedures.

Requirements

  • Current registration with the Australian Health Practitioner Regulation Agency (AHPRA).
  • Strong clinical skills and knowledge, with a focus on aged care and chronic conditions.
  • Excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare professionals.
  • Ability to work independently and as part of a team, with a strong focus on collaboration and teamwork.
  • Knowledge of relevant legislation, policies, and procedures, including the Aged Care Act and the National Safety and Quality Health Service (NSQHS) Standards.
