Job Title: Health Promotion Officer

This role is responsible for leading the promotion and awareness efforts for our innovative Telehealth Cancer Nursing and Navigation Program.

About the Role
  • We are seeking an experienced professional who has exceptional customer service skills, an empathetic personality, and a strong commitment to delivering high-quality patient care.
  • The successful candidate will report to the Head of Patient Programs and be responsible for providing personalised support and guidance to individuals diagnosed with rare cancers, addressing their unique needs, concerns, and challenges.
  • The role involves assisting patients in navigating the healthcare system, including coordinating appointments, accessing financial assistance programs, and understanding treatment options.
  • It also includes educating patients and their families about their cancer diagnosis, treatment options, and available support resources.
  • The candidate will be required to stay informed about advancements in rare cancer research, treatment modalities, and supportive care services to provide up-to-date information to patients.
  • Additionally, the role requires offering compassionate listening and emotional support to patients and their families as they navigate the challenges of a rare cancer diagnosis.
  • The successful candidate will also provide referrals to counselling and coping strategies to help patients cope with the emotional impact of their diagnosis, treatment side effects, and uncertainties about the future.
  • The role involves collaborating with healthcare providers, multidisciplinary cancer care teams, and community organisations to coordinate comprehensive patient care plans.
  • Enhancing support services for rare cancer patients by building and maintaining relationships with key stakeholders, including medical professionals, researchers, patient advocacy groups, and government agencies.
  • Maintaining accurate and confidential records of patient interactions, including assessments, referrals, and outcomes, in accordance with privacy and confidentiality policies.
  • Generating reports and data summaries to evaluate the effectiveness of patient support programs and inform strategic planning and decision-making.
Requirements
  • Bachelor's degree in a relevant field, such as health sciences, nursing, or public health.
  • At least 3 years of experience in a related field, preferably in a patient support or advocacy role.
  • Exceptional customer service skills and an empathetic personality.
  • Strong communication and interpersonal skills.
  • Ability to work effectively in a team environment.
  • Proficiency in Microsoft Office Suite, particularly Word, Excel, and PowerPoint.
  • Knowledge of rare cancer research, treatment modalities, and supportive care services.
  • Familiarity with patient navigation and coordination.
  • Ability to maintain accurate and confidential records.
