We are seeking a skilled professional to work in the community, providing high-quality care to residents in Residential Aged Care Facilities.
Key Responsibilities:- Work as a Registered Nurse with endorsements to practice as a Nurse Practitioner.
- Provide clinical expertise and support to patients in the RACF setting.
- Develop and implement care plans that cater to the unique needs of each patient.
- Current registration and endorsement as a Nurse Practitioner with relevant health authorities.
- Practicing certificate.
- Relevant qualifications in gerontology, palliative care, or chronic disease management (desired).
