We are seeking Registered Nurses and Enrolled Nurses to join our clinical team across multiple sites in Victoria.

As a key member of our team, you will provide high-quality nursing care to patients in their place of residence within the community.

This is a great opportunity to work with a dedicated and professional clinical team who deliver excellent standards of care to patients throughout the community.

Key Responsibilities:

  • Assess and document the anticipated needs of the patient.
  • Make relevant referrals as necessary.
  • Assess the occupational health and safety needs in the home and organize occupational therapy assessment to improve safety if necessary.
  • In accordance with the prepared nursing care plan, provide care that encourages the return to independent living where appropriate.
  • Liaise closely with the Registered Nurse, Clinical Case Manager and treating Doctor to maintain the highest possible standard of nursing care within the home.
  • Observe, record and report to a Registered Nurse specific and general physical and mental conditions of patients, and symptoms and signs which may be indicative of change.

Requirements:

  • Valid State Drivers Licence.
  • Registered with the Nursing & Midwifery Board of Australia (AHPRA) as a Registered Nurse or Enrolled Nurse.
  • Registered Nurses must have minimum three years post graduate experience in acute nursing or speciality.
