About the Job
We are seeking a skilled Senior Social Worker to join our multidisciplinary team. As a key member of our team, you will be responsible for working across inpatient and outpatient services with vulnerable people on complex safety and discharge planning.
Key Responsibilities:
- Collaborate closely with a multidisciplinary team to provide high-quality patient care.
- Build strong referral pathways to local NGOs to ensure seamless patient transitions.
- Develop a breadth of skills by rotating across clinical areas such as medical, surgical, rehabilitation, paediatrics, and maternity and special care nursery.
