We are seeking a highly skilled and experienced Social Worker to join our team on a remote island off the coast of Australia.

This is a unique chance for an individual to work in a multi-purpose facility providing care and support to clients with diverse needs.

The successful candidate will be responsible for delivering high-quality social work services, including assessment, planning, and intervention.

Key aspects of the role include:

  • Providing person-centered care to individuals in a small aged care facility and acute setting
  • Working with clients who have complex needs, including those related to domestic violence, grief, and loss
  • Developing and implementing individualized care plans that promote client well-being and independence
  • Collaborating with healthcare professionals to ensure seamless care transitions and outcomes

The ideal candidate will possess strong social work skills, experience working in hospital settings, and a commitment to delivering high-quality care.

Essential Requirements
  • AASW eligibility as a registered Social Worker
  • Strong background in aged care, with experience working with clients 65+ years old
  • Experience in generalist social work, including domestic violence, mental health, and case management
  • National Police Check or willingness to obtain one
  • Covid-19 vaccinated
