We are seeking a proactive and energetic Support Worker to join our Shared Living division in Concord West. As a key member of our team, you will play a vital role in supporting and empowering people with disabilities in their Supported Independent Living (SIL) accommodation.

About the Role

As a Support Worker, you will be responsible for providing support and assistance to clients in their group accommodation during active hours. You will also be on-site overnight and be expected to support during the inactive/sleep break when necessary.

Key Responsibilities:

  • Support clients in their daily routines, including personal care, medication administration, and household tasks.
  • Assist and support clients with their routines, which may include laundry, person-led activities in their homes and out in the community.
  • Maintenance of accurate progress notes and incident reporting to meet organizational and NDIS requirements.
About You

We are looking for an individual who is an adept team player, with experience supporting people with complex behaviors and health needs. You will have demonstrated experience following client-focused plans, including behavior support plans, medication management plans, and other health-related plans.

You will be required to have a minimum Certificate III in Disability, Individual Support, or Community Services (or similar). You will also be expected to maintain Sunnyfield's values: Respect, Trust, Honesty, and Innovation.
