We are seeking a skilled and experienced Nurse/Midwife to join our team as a Diabetes Care Coordinator. This role provides critical professional advice and support in the assessment, planning, review, evaluation, and coordination of client care in the defined practice area.
Key Responsibilities:
- Provide expert advice and guidance on diabetes care and management
- Develop and implement care plans to meet the needs of clients with diabetes
- Collaborate with healthcare teams to ensure seamless care coordination
- Conduct regular reviews and evaluations to assess client progress and adjust care plans as needed
Requirements:
- Current registration with the Nursing and Midwifery Board of Australia
- Graduate Certificate in a relevant specialty area
- Experience driving a manual vehicle and eligibility for a NT Drivers Licence
- Willingness to travel to remote communities by 4WD or light aircraft
